What can I do to prevent this? Door is aligned tight. Old weatherstripping around the window? It doesn't have the plastic covering for the blowout clip, but the clip is there.
Has anyone fixed this problem?

I would think you need the plastic piece on the blowout clip. It has a bump that pushes on the window.
 
try adjusting the window pin that interlocks with the nylon loop thats on the rear of the glass (inside the door) ..move it outward
you need to remove the upper door panel to get at it
you can also adjust the rear vertical track at the top of the door , move it outward toward glass to push glass against dew strips which makes the glass move toward weatherstrips
also make sure your dew strips arent worn out
