scanmaster "code" sheet

Does anyone know where I can get a copy of the scanmaster "code" sheet... the one that comes when you buy a SM2.

I thought there was a copy on GNTTYPE.ORG but I can't find it. My 86TTYPE is running like crap and I want to do some quick monitoring.
