It's a B&M Streetstick. It's no longer produced because there wasn't enough market for it. 
It was designed especially for Turbo Regals, and Monte Carlo's back in 87-88 time frame. So yes it came with a custom plate that has a lighted, sliding gear indicator, etc.
The only things that are available now are generic B&M Prostick's or Hurst Quartersticks. You'll have to come up with your own console plate.
