Fan activation does not always work

Is there something special about the fan activation? I have it set to say 190* on and 180* off, the engine will get to well over 200* and the fan still doesnt turn on some times. This doesnt look like a verry consistant problem, as it works some times, and doesnt at others.

I am running a bank 2 bank on a 99 Dodge Dakota.

That should be it.

FWIW, sometimes I've had a bad ground (moisture sensitive), so the ECU told the fans to come on (indicated on the dashboard) but they weren't actually moving. Also, don't use a seperate temp gauge- only use the temp indicated by the ECU as the judge.
