Anybody break a pin extender before?

Long story short it's the first and last time I'll ever use one! But it was a ck servo and even with the longest stock pin and shimmed it still fell short of ideal clearance. I went back in the trans for other reasons and found it broken. It didn't do any damage but band clearance was well over .200

I have used them and never seen one break, could it have been ground down too much? It looks small in the pic
