Need an ultrachip spec sheet PDF file

I am trying to send in my ultrachip to have it reprogrammed but caspers has actually misplaced the sheet. I thought I would see if anyone has the PDF file saved to their computer by chance.

I actually need the sheet that you fill out before you purchase the chip not what the different settings are.

If anyone has this PDF file I would appreciate it if you could email the file to me at mccjazr@aim.com.

Did you ever find what you need. I happen to have paper work for the Ultrachip. I see it's been a few years so i'm sure you're done.
