Tranny cable popping out of bracket

I've got a reoccuring problem of the tranny cable popping out of the bracket that is just below the drivers seat under the car. The cable is supposed to sit in the bracket with a retaining clip, but,,,, this clip keeps popping out when ever I drive the car. Any one have this problem and how did you solve it?

1) Clip is worn (replace)
2) Your installing the clip upside down
3) Slot in cable sheath is worn
4) Cable grossly missaligned with shift lever on trans
