Looking for a PCI Voice/fax//data modem with speaker phone

Greetings all,

I am looking for a PCI Voice/fax//data modem with speaker phone. With the mic. and headphone 1/8" plugs on the card it self.

Any suggestion , new - used doesn't matter has long as it works.
