I've had it off and on since my early twenties, sometimes one side, sometimes the other and sometimes both sides.
I tried everything. Shots, surgery, ice, heat, physical therapy, you name it.
I discovered this about 6 years ago when a woman who works where I do heard me snivelling about it.
she said "Have professional masseuse massage your forearm in the little "V" between the muscles on top, beleive me you'll know when they are on the spot that's causing the pain."
Have them dig in on it real good for about ten minutes. It'll hurt like hell but in the morning you'll be thanking that person. Yeah, you can try the wife/GF route for the massage, but they will quickly grow tried of it.
Anymore, I just go to Massage Envy at the first twinge of it and pay $65-75 for someone to just maul my arm, shoulder and back on the side that's starting to act up. It usually fixes me up for about a year and a half.
If you're like me and sleep with your arm extended upward under the pillow and your head, buy a good quality wrist brace and when the pain starts to come on, wear it when you're sleeping. You'd be amazed at how badly you can hurt yourself when your sleeping.
