You CAN do a lot of things manually, but spybot does have a nice host file insertion tool that prevents access to certain known domains... that and the immunize feature works pretty well.
Also, a kinda cool thing is http://www.siteadvisor.com/ which can either be used manually for lookups or as a plug-in for your browser. It actually blocked access to a site I clicked on today. REALLY cool to see a rating on your google results too.
