I would check the puck. TH puck size is 1 3/8" in size. The actuator port he is 1 4/8ish". So all in turn,
1. your puck is letting boost pass by and not building boost. Then all of a sudden you have a boost creep/spike from the amount of pressure.
2. That pressure caused can cause your inlet hose to pop off and detonate.
That is why that RJC makes a bigger puck, 1 5/8", and a block off plate that you drill out to match your puck. He also makes a flange with the puck already installed. So if you know a good tig welder then there you go. I know several people, including myself (have THDP) on booth of my cars. THDP was and is still a great DP. Just back then before the TE turbos the actuator hole was not that big. Now if you search the forum for TH puck, you will read all about sudden boost creep/spike. Just saying. Who am I, not one of the big dogs on this site with attitude.
